Important information regarding treatment

Thank you for your interest in IPM Medical Group, Inc. All new patients being referred to our facility must have a written referral, regardless of insurance, from a physician, adjuster or attorney. The written referral should include the following information:

  • The written referral with a diagnosis and reason for referral
  • Medication history
  • Most recent visit notes
  • Authorization if required. (All HMO insurances and Workers' Compensation referrals need authorization)
  • Insurance information
  • Demographic information

This information can be faxed to (925)691-9807. You can also have your doctor use our New Patient Referral Form. Once received, our office will contact you for scheduling. Please allow time for review and processing. If you would like to check the status of your referral, or to schedule your appointment after your referral has been sent, please contact the new patient scheduling department at (925)691-9806 ext. 105 or 135.

If you are being referred by an attorney or chiropractor for treatment following a motor vehicle accident, download our Motor Vehicle Accident Questionnaire. Otherwise, download our New Patient Packet.

There will be additional paperwork that will need to be completed and/or signed once you check in. Please plan to arrive 15 minutes prior to your scheduled appointment time to allow for completion of these forms. If you have not completed your packet, please plan to arrive 45 minutes prior to your scheduled appointment time. If you do not have your packet completed and do not arrive early to complete it, your appointment may need to be rescheduled.

What to bring to your first appointment:

  • California Identification Card or California Driver License
  • Your insurance card
  • Your new patient packet. If you did not receive your packet in the mail and are unable to download the forms above, please plan to arrive 45 minutes prior to your scheduled appointment time to complete the forms in our office.
  • Current prescriptions (prescription bottles are preferred but a current printout from your pharmacy will also be accepted)
  • Your co-payment. Payment is due at the time of registration. Please note that we accept cash, money orders or credit cards only. We do not accept checks as payment for any type of appointment or service.

What to expect during your first appointment
You should plan to spend about an hour and a half during your initial appointment. This includes arriving 15 minutes prior to your scheduled appointment time to complete registration.

Your initial evaluation is your first meeting with our physician so please be prepared to provide information regarding previous treatments, physicians, studies and tests. The doctor will discuss treatment options with you and if we are authorized for additional visits beyond the initial consultation, we will attempt to obtain authorization for these treatment recommendations. Your referring physician/insurance carrier/attorney will be sent a copy of your consultation report as well as all future visit notes.

Important Information

  • Please provide at least 24 hours notice if you need to cancel or reschedule your new patient appointment. Failure to provide sufficient notice may result in fees applied to your account that may need to be paid prior to rescheduling.
  • You must be seen at least one time every six months. After six months, a new referral is required and you will have to be scheduled as a new patient.
